Care Access and Community Navigation Program
The Care Access and Community Navigation Program helps individuals and families overcome the social and environmental challenges that impact their health and stability. Our team provides hands-on support with housing navigation, food access, transportation coordination, digital literacy, benefits and entitlement assistance, and linkages to trusted community resources. We focus on building strong relationships, improving daily stability, and helping clients stay connected to the care and services they need. Through culturally responsive support and person-centered problem solving, we reduce barriers, promote empowerment, and strengthen long-term well-being for the communities we serve
